Heritage Middle School/YMCA Sports Program

The West Essex YMCA and the Heritage Middle School are ready to host the sports season once again.  2016-17 school year will be the 7th year we are hosting Middle School sports and each year there have been wonderful accomplishments by players and coaches alike.  

We are looking forward to another great year and encourage students to try out for any of the sports listed below. Practices will commence as soon as teams are established, schedules will be handed out by the coaches and posted on our website as soon as they are available.  Remember being part of a team has commitments, attendance at practices, making the scheduled games, (unless ill or excused by the coach), maintaining a solid grade average, being cooperative and respectful to all.  

There is a participation fee for each sport of $250.00, please make checks payable to the West Essex YMCA and bring to the coach during try outs.   If your child is not selected for the team your check will be returned to you by mail. The fee for Cross Country team is $150.00.  Financial assistance is available for those with special circumstances, all inquiries are kept confidential.   Information may be found on the “HMS/YMCA Athletics” link on the Heritage Middle School website or directly on the West Essex YMCA website under Youth Development. Here is information on how to sign-up for sports

Students wishing to participate in an interscholastic sport, intramural sport and/or club program (if program requires a physical-check with the school office) must complete the appropriate forms in the Genesis Parent Access portal:

  • HMS/YMCA Athletic Permission Form (for all seasons)
  • Intramurals/Dance Club Permission Form (for all seasons)
  • Athletic Emergency Card (completed once for the current school year) 
  • Athletic Policies and Information Form (completed once for the current school year).  This form includes:
  • Random Steroid Testing Consent
  • Acknowledgement of reading the Concussion Policy
  • Acknowledgement of reading the “Sudden Cardiac Death in Youth Athletes” brochure
  • Acknowledgement of reading the LSC Sportsmanship Pledge

Sports Physicals  

Pre-participation Physical Evaluation form is required by the State of New Jersey for students in grades 6-12 participating on a school-sponsored interscholastic or intramural athletic team or squad and must be completed prior to a student’s participation as per N.J.A.C. 6A:16 Programs to Support Student Development.  

A Health History Update Questionnaire is required if the athletic physical on file is older than 90 days prior to the first official practice.

Included in the State of New Jersey mandate regarding participation in sports, physicals are valid for one calendar year only.  All physicals MUST be documented on the official State of New Jersey physical form. 

IMPORTANT NOTE:  If a student has asthma, an Asthma Treatment Plan must be on file in the student's health records.  If you have not already done so, please have an Asthma Treatment Plan form completed and return it along with all other paperwork.  

Sports physicals may be completed by a student’s personal physician or if you cannot afford to have a physical examination done or do not have a family physician, school physicals are offered prior to each season.  These physicals are performed in the Health Office at Livingston High School.

*Please bring the printed Pre-Participation Physical Evaluation Form and filled out Health History Questionnaire*


If your personal physician is administering the physical examination, his/her findings must be recorded on the official Pre-participation Physical Evaluation form.  Please be sure to complete all appropriate sections and have a parent/guardian sign the form prior to visiting your physician.

The due date for submission of all LPS forms is the same date that the season's physicals are given.
